Pharmacists are the only health care providers formally trained in the art and science of compounding medications. Therefore, pharmacists are expected, by the medical community and the public, to possess the knowledge and skills necessary to compound extemporaneous preparations. Pharmacists have a responsibility to provide compounding services for patients with unique drug product needs.
Currently we provide only simple and moderate non-sterile compounding services to our community.
We make preparations that have a United States Pharmacopeia (USP) compounding monograph or that appears in a peer-reviewed journal article that contains specific quantities of all components, compounding procedure and equipment, and stability data for that formulation with appropriate Beyond Use Date (Expiration Date); or reconstituting or manipulating commercial products that may require the addition of one or more ingredients as directed by the manufacturer.
We conduct a thorough medication therapy review (MTR) through the following guidelines:
• We employ active communication skills to overcome communication barriers.
• Complete a personal medication record (PMR).
• Develop a medication-related action plan (MAP).
• Document medication therapy management (MTM).
• Create a plan for patient follow-up.
• Assess patient-specific data (including lab reports, Genetic Typing) and issues, and interpret these findings to identify medication-related problems.
• Develop a patient problem list and a list of the patient’s medication-related problems.
• Establish goals of therapy for each medical condition and medication-related problem identified.
• Make recommendations for resolving situations in which a patient’s medications are potentially inappropriate.
• Discuss the elements of an effective medication therapy recommendation to another health care practitioner (PCP, Specialists, etc.).
• Describe the medication use, concerns and adverse drug events that are more common in elderly patients.
• Account for pharmacokinetic and pharmacodynamics changes when reviewing pediatric and elderly person’s medication regimen.
This review process ensures you are are provided the most effective medication in its barest form, to be taken over the shortest time possible with the smallest possibility of side effects for your individual condition.
Medications are compounded or prepared by our pharmacists at the request of a prescriber to:
1- Create a patient specific dosage form (Example; capsules, suspensions, suppositories, creams, ointments, gels etc.) for a medication which is not commercially available.
2- Reformulate a medication without the color, dye, preservative/stabilizers which may be allergic to a particular patient.Combine several individual medications into one to dramatically improve patient compliance.
3- We will gladly reformulate or create a whole new preparation to make administration easier. For example we reconstitute to remove dyes, colors, preservatives etc. that may pose as allergens to a customer.
We make preparations that have a United States Pharmacopeia (USP) compounding monograph or that appears in a peer-reviewed journal article that contains specific quantities of all components, compounding procedure and equipment, and stability data for that formulation with appropriate Beyond Use Date (Expiration Date); or reconstituting or manipulating commercial products that may require the addition of one or more ingredients as directed by the manufacturer.
We also prepare formulations that require special calculations or procedures (such as calibration of dosage unit mold cavities) to determine quantities of components per preparation or per individualized dosage units; or making a preparation for which stability data for that specific formulation are not available.
